MAC Pharmacology Diuretics #2 Unit 25

Osmotic diuretics
are agents capable of being filtered by the glomerulus, but have limited capability of being reabsorbed into the bloodstream

Osmotic diuretics indications
prevent acute renal failure during trauma, prevent cerebral, cebebrospinal, intraocular pressure during trauma/surgery/disease/acute glaucoma

Osmotic dosage
1-2 g/kg at one time; additional dosage of 500 mg/kg every 6 hours if needed

Osmotic diuretic patient care
increase fluid 2-3 L per day
sucking on ice chips or hard candy relieves thirst

Osmotic diuretic adverse reactions
urinary retention, convulsions, thirst

Potassium Sparing diuretics
inhibits exchange of sodium for potassium in the distal tubule; inhibits potassium excretion

Carbonic anhydrase inhibitor diuretic
increases the excretion of bicarbonate ion, which carries out sodium, water, and potassium

Interstitial Cystitis
called painful bladder syndrome, bladder pressure, pain, and pelvic pain

Women
90% of interstitial cystitis are women and live with chronic pain

interstitial cystitis patient teaching
drink plenty of water, drink water before sex, urinate after sex

Rimso-50
medication administered directly into the bladder for treatment of interstitial cystitis

Signs of nephrotoxicity
oliguria (production of small amounts of urine), proteinuria (protein in urine)

UTI symptoms in Adults
urgency, pyuria, burning & pain during urination, hematuria, pain/spasm in bladder / pelvic area

UTI symptoms in Children
frequency, pain during urination, abdominal pain, hematuria, bedwetting (enuresis), fever, chills, flank pain

UTI symptoms in Infants
fever, loss of weight, nausea, vomiting, increased urination, foul smelling urine, diaper rash, failure to thrive

Sulfonamides
among the drug of choice for acute, uncomplicated UTI, especially E coli and Proteus mirabillis bacterial strains

Escherichia coli
type of bacteria that is commonly found in the alimentary tract of humans & animals

Proteus Mirabillis
species of enteric bacilli that may cause UTI, found in intestines of humans and animals

Sulfonamides classification
can be classified according to length of time they remain in the body

Sulfonamides grouping
Short (used in treatment of UTI, rapidly absorbed, produce high level of drug in urine; intermediate; long – lasting

acetazolamide (Diamox) – Indications Carbonic anhydrase inhibitor. open-angle glaucoma, pre-op treatment of closed-angle glaucoma, seizure disorder, edema, high-altitude sickness. acetazolamide (Diamox) – MOA Inhibits carbonic anhydrase in kidneydecreases water reabsorption, increases Na+ & K+ excretion. Decreases aqueous humor production. WE …
